We stand at the edge of a transformative era. Artificial intelligence is beginning to show what it can do in medicine—not just as a tool, but as a partner in diagnosis, reasoning, and care. The MAI-DxO system, introduced in the Microsoft-backed study Sequential Diagnosis with Language Models, shows how an orchestrated, truth-seeking AI can outperform even experienced physicians in both accuracy and cost-efficiency.

This is not incremental progress. This is a signal that the foundations of medicine are about to be rewritten.
But as with any powerful system, the question is: who controls it?
The Capture Problem
Today’s healthcare infrastructure is built not on truth, but on profit. Pharmaceutical giants, insurance monopolies, and opaque medical networks have shaped an industry that often values billing codes over healing. Now, they seek to capture the next great tool: AI.
This is no abstract risk. Regulatory capture—where supposed watchdogs become gatekeepers for industry—already distorts health policy, pricing, and access. As AI enters the scene, we risk repeating this mistake at a deeper, more dangerous scale.
Even more troubling is the entanglement between Big Pharma and mainstream media. When pharmaceutical companies supply the majority of advertising revenue for major media outlets, what incentive does the media have to question their motives? If AI systems threaten to expose simple, low-cost interventions—like changes in diet, sleep, lifestyle, or natural remedies—that could reduce dependency on lifelong pharmaceutical regimens, what’s to stop those industries from shaping or censoring the outputs of AI itself?
They’ve made trillions keeping us on daily medication. It is not in their financial interest for us to be truly well. It is more profitable to treat disease than to prevent it. It is more profitable to prescribe than to empower. And they have the money and the influence to defend that business model—even if it means turning AI from a liberator into a controlled narrative device.
Alignment Means Outcomes
We must reclaim the meaning of “alignment.” In the world we’re building, alignment does not mean compliance with corporate incentives—it means alignment with life.
- Alignment with health, not hospitalizations.
- Alignment with longevity, not lifelong subscriptions to treatment.
- Alignment with truth, not marketing.
- Alignment with happiness, not quarterly returns.
MAI-DxO shows the incredible diagnostic power of an AI system allowed to reason, revise, and think iteratively. But that same power, in the hands of the wrong actors, becomes a tool for manipulation, dependency, and profit optimization.
The TOBIKO Declaration
This is why we are building TOBIKO as a Public Benefit Corporation.
Our mission is not profit. Our mission is to build systems that are morally guided, publicly accountable, and structurally aligned with human flourishing.
- We will state our goals publicly and design toward them.
- We will measure success not in revenue, but in outcomes: real healing, real empowerment, real understanding.
- We will build a team of collaborators who believe that health is sacred, and data is sovereign.
Your Data, Your Destiny
At the center of this movement is a simple principle: your data belongs to you.
We must create systems where:
- Individuals own and control their health data.
- They can choose to contribute it into Data unions ( aka Dividends ) that serve their own goals (e.g. “help me solve my autoimmune condition”).
- They are protected by consent frameworks stronger than HIPAA—dynamic, revocable, granular.
- AI systems trained on that data are owned by the people, not by corporations.
This is not just about privacy. This is about agency, freedom, and dignity.
What Is a Data Dividend?
A Data Dividend is a new kind of data structure—a collective framework that enables individuals to:
- Choose the destiny of their data: not just whether it is used, but how, by whom, and for what purpose.
- Grant use based on alignment: e.g., “Only use this data for autoimmune research” or “Only if used to benefit open-source medical AI.”
- Revoke access at any time, under any condition, if the data is being misused, commercialized, politicized, or distorted.
Unlike legacy health systems where data flows invisibly between entities, Data Dividends put power visibly back into the hands of the individual. They enable consent as a living, evolving relationship—not a one-time checkbox.
But to unlock this future, we must do more than build systems—we must educate the public.
People must be awakened to their rights, their power, and their voice in shaping these systems. Because unless individuals understand that their data is power, that power will always be stolen by those with the tools to exploit it.
The truth must belong to the people—not to power players trying to suppress, shape, or sell it.

Privacy-First AI for a Decentralized Future
This is why TOBIKO is focused on building privacy-first AI systems.
We are committed to:
- Local AI: Computation that stays close to the user—on-device, on-edge, in your hands.
- Quantum-resilient encryption: Architectures designed to withstand even the next generation of data threats.
- Beyond HIPAA and COPPA: True self-sovereign control of your personal health signals, not just regulatory checkbox compliance.
We believe that the intelligence of the swarm—not top-down control—is the key to navigating health complexity. We look to nature, ecosystems, and emergence as our design language.
A balanced system is one in which no single actor can distort the signal, where clarity emerges from diversity, and where the truth is collectively revealed, not centrally dictated.
The End of the Old Order
It is time for the legacy institutions—the ones who built the current model of extractive, profit-driven medicine—to step aside. They had their turn. It is humanity’s turn now.
With the right architecture, AI can become a force of liberation:
- Helping us understand our own bodies.
- Solving rare and chronic diseases.
- Empowering every person with access to world-class reasoning, not just those who can afford concierge care.
But this only happens if we detangle the tentacles of profit from the system before it takes hold.
An Invitation
We are not building this alone. TOBIKO is a call—to builders, researchers, ethicists, doctors, caregivers, and patients.
If you believe that health should be free from corporate capture…
If you believe that AI should serve the human spirit, not the quarterly report…
If you believe that the future of medicine is yours to shape—
Then join us.
Because it’s your data.
And it’s your destiny.
